Lilly Beat Estimates by $1.8 Billion. The Third Raise Matters More.

Mounjaro and Zepbound topped $16B in Q3. Is it priced in?

Wall Street spent Thursday night selling tech stocks into the ground. Eli Lilly closed up more than 4% in premarket Friday. That divergence alone explains why LLY deserves the most attention in this market right now.

The Business

Eli Lilly makes two drugs that are, at this moment, reshaping how pharmaceutical companies are valued. Mounjaro is an injectable GLP-1 treatment approved for type 2 diabetes. Zepbound is the obesity indication of the same active compound. Both are growing faster than almost any product in the industry’s recent history, and both continued to accelerate in Q3.

The company also runs a growing oncology, immunology, and neuroscience portfolio, but Mounjaro and Zepbound are driving the majority of the growth story investors are watching.

Why Wall Street Is Paying Attention

Revenue rose 39% from a year earlier to $24.4 billion, beating consensus by about $1.8 billion. Non-GAAP earnings came to $7.48 per share, well above estimates. Those are not narrow beats; they represent a company consistently outrunning what analysts think is possible.

Lilly raised its 2026 revenue forecast to $91 billion to $92 billion after third-quarter revenue climbed 39% to $24.4 billion. The company’s previous range was $85 billion to $87 billion. It is the third guidance increase this year. That cadence matters as much as any single quarter’s number.

What’s Driving the Opportunity

Mounjaro revenue rose 65% to $10.7 billion, and Zepbound revenue rose 49% to $5.35 billion. Combined, the two drugs generated more than $16 billion in Q3 alone. Internationally, the story is only getting started. Lilly’s international incretin business increased 53% year over year based on IQVIA gross-sales measurements, and the company’s share reached approximately 58%, up nearly four percentage points from the second quarter.

Key Products revenue in the Immunology, Oncology, and Neuroscience therapeutic areas grew 100% in Q3 2026 compared to Q3 2025. That doubling tends to get overlooked when Mounjaro and Zepbound dominate the headline, but it signals the business is not a two-drug platform. It is a pipeline showing up.

What Could Go Wrong

The price-decline question is real. Lilly saw a 41% increase in volume for its products, primarily Mounjaro and Zepbound, partially offset by a 7% decrease in realized prices, primarily driven by Zepbound. That spread between volume and price will widen if rebate and formulary pressures intensify. Medicare expansion into GLP-1 coverage is a tailwind for volume but may add further downward pressure on net pricing.

Competition from Novo Nordisk remains constant. Any efficacy data favoring a rival drug, or any supply advantage that closes the current gap, could shift prescribing patterns faster than the quarterly numbers reflect. There is also valuation risk baked into a stock that has priced in significant continued outperformance. One quarter that merely meets estimates could read as a miss.

The Bottom Line

Lilly raised full-year guidance by a $5.5 billion midpoint increase in a single revision. That is not rounding-error optimism; it reflects genuine demand the company did not fully anticipate three months ago. In a session where the Nasdaq fell 2.9% on AI-spending doubts, Lilly proved the pharmaceutical GLP-1 trade has its own momentum, independent of the tech cycle. The stock is not cheap. But neither is the growth, and on a day when most of the market is retreating, LLY is one of the few places where the fundamental case got stronger, not weaker.
