Health Insurers Face a Second Premium Shock. UNH Still Makes Sense.

With ACA open enrollment starting Sunday and 2027 premiums rising another 15%, the managed care sector's pain is priced in unevenly, and UnitedHealth stands apart.

Open enrollment for 2027 ACA coverage begins Sunday, November 1, and the numbers heading into it are not kind. Premiums are up again: ACA marketplace insurers proposed a median 15% increase for 2027, based on KFF’s analysis of 276 insurers across all 50 states and DC. That follows roughly a 20% increase in premiums for 2026, meaning marketplace premiums are on track to have climbed by more than a third in just two years.

The Damage Already Done

Before asking which managed care stock deserves capital today, understand what the first year of the subsidy cliff already produced. Millions of enrollees saw their net monthly premiums jump substantially for 2026, with premium payments rising by an average of 58% (from $113 to $178 per month). KFF projects average effectuated enrollment could fall from 22.3 million in 2025 to about 17.5 million in 2026, and the Urban Institute estimates 7.3 million people lose subsidized marketplace coverage, of whom 4.8 million become uninsured entirely.

The drop was concentrated: consumers above 400% of the federal poverty level made up just 7% of 2025 enrollment but nearly half (48%) of the decline in plan selections from 2025 to 2026. That is adverse selection in motion. Healthier, higher-income people leave first. Sicker enrollees stay. Premiums rise to cover the remaining pool. Higher costs push healthier individuals to drop coverage, creating adverse selection that drives prices up even further for those who remain. The 2027 enrollment season, with a key deadline of December 15, 2026 for coverage starting January 1, 2027, is about to ratify whether that spiral deepens.

Health plan participation is also shifting. As of October 7, 2026, KFF tracked 11 carriers that have announced they will exit ACA Marketplaces in plan year 2027, either in some or all states where they currently offer plans. The exits narrow choices, reduce price competition in affected markets, and push more cost into remaining carriers.

Why UNH Stands Above the Sector

Most managed care names spent 2025 and early 2026 being punished for medical cost overruns. The recovery has been uneven. Year to date, Centene is up roughly 72% and UnitedHealth is up about 27%, while Molina is up less than 20% and carries a Hold from analysts. That divergence matters: CNC’s run largely reflects a recovery from deeply depressed lows after earnings collapsed; UNH’s gain is built on a genuine operational turnaround.

UnitedHealth issued Q3 2026 earnings on October 13, reporting an adjusted EPS of $4.69, topping consensus of $4.14 by $0.55. Its medical care ratio came in at 88.4%, improving from 89.9% a year earlier, driven by medical cost management, benefit design changes, and pricing discipline. The company now expects full year 2026 adjusted net earnings of at least $20.50 per share. UnitedHealth has raised its 2026 profit outlook three times this year on improving medical costs, while Elevance also lifted guidance and targets a return to at least 12% EPS growth in 2027.

The Optum arm gives UNH something CNC, MOH, and ELV cannot fully replicate: a cost-management engine that sits inside its own business. Its insurance operations are complemented by Optum’s data, pharmacy, and care delivery platforms. When claims costs rise across the industry, Optum’s scale in care delivery provides a partial offset unavailable to pure-play insurers.

What Could Go Wrong

The ACA marketplace is not UnitedHealth’s primary revenue driver, which is part of the case for it. But the adverse-selection spiral heading into 2027 enrollment is a sector-wide problem, and no managed care company is immune to rising utilization in the pools they insure. Analysts at Raymond James and Baird both lowered their price targets on UNH following Q3, to $450 and $427 respectively, even as both maintained positive ratings.

Policy risk is real and binary. The November 3, 2026 election is nine days away, and subsidy restoration is on the ballot in ways that could reshape enrollment economics in either direction. A Congress that restores enhanced premium tax credits would bring enrollment back, reduce adverse selection pressure, and likely benefit carriers still active on exchanges. One that does not will accelerate the spiral through 2027. Enhanced premium tax credits have not been restored as of this writing.

The Bottom Line

UNH closed at $418.09 on October 23, trading at roughly 20 times its raised 2026 earnings guidance. That is not a distressed valuation, but it is reasonable for the sector’s most defensible franchise. Elevance carries lower multiple expansion risk given its 2026 Medicaid trough, and Molina’s analyst consensus has dimmed. CNC’s YTD surge has compressed its margin of safety.

Among managed care names, UNH offers the strongest combination of operational momentum, diversified revenue through Optum, and demonstrated ability to manage pricing faster than peers. The 2027 ACA premium environment is a headwind for the sector. For UNH, it is a manageable one.
